Three things caused it: a search that kept rewording itself until no room was left to answer, a tool call the model wrote as plain text so nothing ran, and a length cap that cut reasoning models off before they spoke. Search now steps aside after three rounds, a call written as text still runs, the cap is gone — and a round that still ends silent is retried once, then kept in Recent marked Failed instead of vanishing with your question.
